  • What are the registry requirements to receive notice?
    Statute: - § 48.025(2)
    • A putative father can register a parental interest before the termination period of the parental rights of the putative father.
    • This declaration should be signed and written by the putative father.

  • Registry Information :
    Statute: - § 48.025(2)
    • In declaration, you should include:
    • The name with full address of birth mother and the putative father
    • Birth date of child (month and year)
    • A statement

  • Paternity Revocation (of claim):
    Statute: Not addressed.

  • Registry Information Access :
    Statute: - § 48.025(3)
    • This information is sent to the birth mother of child.
