Putative fathers
Yes, this state have a Putative Father Registry.
Registry requirements to receive notice : Statute: 750 ILCS 50/12.1(b)
- The registration have to be written as well as signed by the putative father.
- The putative father has to register before the child birth, but in 30 days after child birth.
Information in the registry or claim : Statute : 750 ILCS 50/12.1(a)
- Date on which the 'Department of Children and Family Services' got the putative father registration.
- Any other information if found necessary.
- Name, address, SSN (social security number) and birth date of putative father and birth mother
- Name, date of birth or adopted date of birth of the child, gender, place of birth etc.

- Any child welfare organization
- Attorney if represents any party
